MEDICAL SERVICES
We believe that health care is more than simply treating illness, but working with you to stay healthy. As a team we are committed to providing quality primary medical care to Santa Clara and San Mateo Counties. We have been serving the community for over 50 years and are happy to be a part of the healthcare delivery system. As healthcare is ever evolving we know the key to success is the patient experience. We at Gardner Health Services strive to keep our patients healthy through a multidisciplinary approach. In order to do that we offer multiple services under one roof. Each patient has the opportunity in one visit to receive care from different disciplines.
Patient-Centered Medical Home
Gardner Health Services strives to provide high quality care. A way in which we are honoring that commitment is through our pursuit of the National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) recognition program. PCMH is a model of care that puts patients at the forefront of the care they receive. PCMH is a primary care model that aims to build better relationships between people and their clinical care teams through accessible, coordinated, comprehensive, and continuous quality health care. The patient’s care team will ensure that the patient’s preventive, acute and chronic health care needs are met by planning, coordinating and managing aspects of the care needed to protect or improve the patient’s health overall.
Under the guidance of this model, Gardner serves as our patients’ medical home by focusing on the below functions and attributes of a PCMH:
• Patient-centered: At forefront of our efforts is relationship-based primary health care that is oriented toward the “whole person.” We recognize that partnering with patients and their families requires understanding and respecting each patient’s unique needs, culture, values, and preferences. As a PCMH, our practice actively supports patients in learning to manage and organize their own care at the level the patient chooses. We believe that patients and families are core members of the care team and ensure that they are fully informed partners in establishing care plans.
• Comprehensive care: We are accountable for meeting the bulk of each patient’s physical and mental health care needs, including prevention and wellness, acute care, and chronic care. As comprehensive care requires a team of health care providers to focus on each patient’s individual needs, each patient is assigned to a care team.
• Coordinated care: As a PCMH, we coordinate care across all elements of the broader health care system, including specialty care, hospitals, home health care, and community services and supports. Such coordination is particularly critical during transitions between sites of care, such as when patients are being discharged from the hospital. Our goal is to excel at building clear and open communication among patients and families, the medical home, and members of the broader care team.
• Access to care: We offer accessible services with the objective to have shorter waiting times for urgent needs, enhanced in-person hours, around-the-clock telephone or electronic access to a member of the care team, and alternative methods of communication, such as email and telephone care.
• Systems-based approach to quality and safety: We demonstrate a commitment to quality and quality improvement by ongoing engagement in activities such as using evidence-based medicine and clinical decision-support tools to guide shared decision-making with patients and families, engaging in performance measurement and improvement, measuring and responding to patient experiences and patient satisfaction, and practicing population health management.

FAMILY PRACTICE
The specialty of Family Medicine is centered on lasting, caring relationships with patients and their families. Family physicians may integrate a variety of medical services to provide continuing and comprehensive health care. The scope of family medicine encompasses all ages, sexes, organ systems, and diseases.
PEDIATRICS
Pediatricians specialize in the care of infants, children and adolescents, or usually up to the age of 18. The word pediatrician is of Greek origin and means “healer of children.” As with adult medicine, a pediatrician can work as a general practitioner or specialize in a specific branch of medicine, but works to build a long term relationship with each patient.
INTERNAL
Internal medicine doctors focus on adult medicine. They study the prevention and treatment of adult-related diseases, and can work as a general practitioner or specialize in a specific branch of medicine; they also work to build a long term relationship with each patient.
WOMEN'S HEALTH
Women’s health utilizes medical professionals with specific knowledge of the female anatomy and expertise in caring for women, including services in Obstetrics and Gynecology. Our Women’s Health Clinic work is arranged to collaborate with the Santa Clara Valley Medical Center (SCVMC) in order to maintain continuous service and aid throughout the cycle of childbirth.
OPTOMETRY & OPTICAL
Our Optometrists are specialists who are trained to examine and determine eye health and vision. Our optometrists are able to diagnose, treat various eye diseases, and prescribe and fit lenses and more that can improve eye vision. Our Opticians are generally limited to fitting and adjusting eyeglasses, which are provided in our health centers.
PHARMACY
We provide professional, affordable, and convenient pharmacy services to our patients. Our pharmacies are staffed with Pharmacists and Pharmacy Technicians who are committed to care for our patients regardless of their financial status or ethnic origin. Our primary services are the filling of prescriptions, advising patients, and dispensing of medications. Our pharmacies accept almost all insurance plans, Medi-Cal, Medi-Cal Managed Care, Healthy Family, Healthy Kids, FPACT, Valley Health Plan, and Medicare Part D. Discount on medications will be offered to low income patients with no insurance.
DENTAL
Gardner Health Services offers preventive and diagnostic dentistry, restorative, periodontal, oral surgery, prosthetic treatment, endodontic (Root Canal Therapy), and wheelchair accessibility at three locations. Gardner Dental serves patients of all ages and those with special needs, and in a compassionate, caring environment for all. Tooth decay is a silent epidemic that is dramatically rising, causing poor overall health, with the results that children and adults miss school or work. This is an epidemic that can be stopped, and our goal is to increase access to high quality dental care to all Santa Clara County residents.
INTEGRATED BEHAVIORAL HEALTH
We at Gardner Health Services understand the interconnectedness between your physical and emotional health. We are pleased to offer services through our Integrated Behavioral Health team of Counselors, Peer Partners, and Psychiatrists working together with you and your Primary Care Physician to improve your overall health and wellbeing. Dealing with stress, depression, anxiety, or a chronic medical illness can be very difficult. If you are struggling with any of these issues, Gardner Health Services’ Integrated Behavioral Health program can help.
PERINATAL
Perinatal nurse practitioners and specialists provide medical and informational support to pregnant women and their families, as well as mothers who have recently given birth.
PODIATRY
Gardner Health Services offers podiatric care. Our goal is to treat our patients as we would treat our family members. We try to provide comprehensive treatment along with the medical department and other services that are available at Gardner Health Services, especially with our diabetic patients. Podiatric service includes prevention, evaluation, diagnosis and treatment of foot and ankle problems including musculoskeletal, biomechanical, and dermatological conditions. We provide medical, biomechanical, and limited surgical services to patients of all ages.

NUTRITIONAL SERVICES & EDUCATION
Nutrition Services provides education as part of a team approach to health care for our patients. Registered Dietitians and Certified Diabetes Educators educate adult and pediatric patients and their families on all diagnoses affected by Medical Nutrition such as obesity, diabetes, hypertension, hyperlipidemia and pregnancy. Our goal is helping families live healthy lifestyles.
WOMEN, INFANTS & CHILDREN
WIC is a free supplemental nutrition program that provides nutrition education and breastfeeding support to help families eat healthy and stay well.
WIC serves pregnant, post-partum and breastfeeding women, infants and children up to 5 years of age. Foster parents, step-parents, grandmothers and fathers with children can also apply…working families welcome!
WIC Provides:
- Food checks to buy healthy foods, such a fruits and vegetables and whole grains
- One on one and group education about nutrition, health and physical activity
- Breastfeeding help and support, including an electric breastpump loan program
- Referrals to health care and other community services
WIC follows a gross (before tax) income guideline. Working families can also apply.

HEALTHCARE FOR THE HOMELESS
Gardner Health Services’ Health Care for the Homeless (HCH) program continues to maintain its excellent relationships with community resource agencies in Santa Clara County and develops new relationships as community services develop and the needs of homeless people change.
We have agreements to deliver health services during hours that homeless people are present at key homeless shelters and service sites from Palo Alto to Gilroy, including EHC/LifeBuilders, InnVision, St Joseph’s, Mountain View Day Worker Center, Bill Wilson Center, Georgia Travis Center, Salvation Army and other locations.
Because of the high concentration of the homeless and homeless service sites in downtown San Jose, the new Gardner Downtown Health Center is expected to increasing the number of homeless we serve from 3,000 to 4,000 individuals and increase the number of clinic visits from 10,000 to 13,000 for individuals and family members who happen to be homeless.
MENTAL HEALTH SERVICES
Adult Program
The Adult Program seeks to prevent premature institutionalization for individuals between the ages of 18 and 59 years of age. The goal is to develop accurate diagnosis and appropriate treatment plans for clients. Participation in the program is by referral from the Santa Clara Call Center (1-800-704-0900). Accepts only Medi-Cal.
Asian Pacific Youth Program (APYP)
APYP provides services to Asian Pacific youth and their families. The goal is to increase their ability to succeed in school and in their family interactions. The program provides culturally/ linguistically specific mental health services through counseling, crisis intervention, medication support, case management and/or community outreach efforts. Participation in the program is by referral from the Santa Clara Call Center (1-800-704-0900) or by school site. Accepts only Medi-Cal.
Cambodian Culturally Specific Family Services (CCSF)
CCSF provides family-focused mental health services to Cambodian Genocide Survivors and low income Cambodians residing in Santa Clara County. Services may include counseling, crisis intervention, medication support, case management and/or mindfulness meditation groups. Participation in the program is by referral from the Santa Clara Call Center (1-800-704-0900) or by referral by friend, family member, or community member. Accepts only Medi-Cal.
Expanded Differential Response Program (EDR)
EDR provides intensive services to families with reports of abuse or neglect based in hopes of keeping the children out of foster care. Services include case management, counseling, parenting support and community referrals. Participation in the program is by referral from the Department of Social Services. There is no charge to the client.
Family Enrichment Program (FEP)
FEP focuses on empowering families with children ages birth to 5 years of age, building healthy attachments between caregivers and children and in building healthy relationships within the family unit and community. Services include case management, home visitations, parent support and therapeutic services. Participation in the program is by referral directly through the Santa Clara County Mental Health Call Center (1-800-704-0900). Medi-Cal, Healthy Families and Healthy Kids are billed. No payment is accepted from the clients.
Family & Children’s Servicse (F&C)
F&C provides services to children ages 6 – 18. The focus is on improving the emotional, psychological and intellectual well–being of children and bridging cultural and generational differences within families. Strengthens a client’s ability to access personal and community resources via counseling, parenting support medication support and 24 hour on-call service. Participation in the program is by referral through the Santa Clara Call Center (1-800-704-0900). Accepts only Medi-Cal.
Family Strength Based Services (FSBS)
FSBS provides services to families to reduce the risk of abuse and prevent further involvement in the foster care system. Services include counseling, parenting support and community referrals. Participation in the program is by referral from the Department of Children and Family Services [DCFS]. Accepts Medi-Cal and/or qualifying clients under the FSBS Grant.
First 5 Gardner Family Resource Center
A First 5 funded program in your community where children, parents, and caregivers can learn and grow together. Family Resource Center offers free resources, educational activities, and parenting workshops for families with children under the age of 6, providing support and developing the skills children and families need for success. First 5 Superior Court Initiatives
First 5 Superior Court Initiatives serves families with children ages prenatal to five (5) who need assistance navigating the complexity of the Santa Clara County Superior Court System. Services are strategic and individualized and include prevention, intervention services for children, developmental screenings and care management. Participation in the program is by referral from Family Court, Dependency Court, Juvenile Probation Court and the FIRST 5 hotline (408-260-3700). No payment is accepted from the Client.
Intensive Up-front (IUP)
IUP Intensive Up-Front Parent Skill Building Services to families identified by the Department of Family & Children Services. The goal is to improve parenting skills, reduce the risk of abuse and prevent further involvement in the foster care system. Short term parent support services include Family Wellness Workshops supported by weekly in-home sessions to assist families in generalizing wellness principles into the home environment. Participation in the program is by referral by the Department of Social Services. No payment is accepted from the Client.
Learning Together Initiative (LTI) For Cambodian Families
LTI is a community-based program, in partnership with First 5 of Santa Clara County that focuses on providing culturally sensitive services to Cambodian families with children age’s birth -5 years of age. The program works to ensure early care and education for school readiness. Participation in the program is by referral by community members (word of mouth). No payment is accepted from the client – services covered entirely by the Santa Clara County First 5 Initiative.
Older Adult Program
This program provides services which promote and support a desire to lead a healthy, independent and fulfilling life. For individuals 60 years plus and their caregivers. A multidisciplinary team of professionals provide various services to clients to include crisis intervention, counseling, case management, medication prescription and monitoring and 24 hour on-call services. Participation in the program is by referral through the Santa Clara Call Center (1-800-704-0900). Accepts only Medi-Cal or Medi-Care.
System of Care Program
System of Care provides psychiatric and counseling services to high risk youth involved in the social services agency and the probation department to improve their daily functioning. Participation in the program is by referral from the Department of Social Services/ Accepts only full scope Medi-Cal.
The Therapeutic Behavioral Services Program (TBS)
TBS provides children and youth, whose behavior is severe enough to risk losing placement or hospitalization, with intensive short-term treatment. To be accepted into the program, the client must be in the system receiving mental health services. Participation in the program is by referral by both internal program and community member. Accepts full Scope Medi-Cal.
Juvenile Probation Department (JPD)
JPD targets youth (wards of the court) who present an array of emotional, behavioral, social and/or environmental stressors. The goal is to assist these juveniles and their families by improving their functioning and by assisting to complete probationary requirements. Services include comprehensive, culturally/linguistically proficient assessments, individual family and group counseling and psychiatric and medication evaluations. Participation in the program is by referral from the Department of Juvenile Probation only. Accepts full scope Medi-Cal only.
Parolee Re-entry Program (PRC)
PRC is one of only six (6) PRC programs funded in the State of California that provides outpatient treatment for newly released prison population. It assists in connecting clients to resources in the community essential to their recovery, wellness and long term success. Services include holistic treatment services via counseling, case management, meditation, exercise and nutrition. Participants are court mandated to enroll in this program. No payment is accepted from the client.
Full Service Partnership (FSP) and Criminal Justice Program (CJP)
FSP/CJP provides intensive outpatient services to clients struggling with dual diagnosis issues. The program seeks to prevent recidivism, mental health and substance abuse relapses. Clients are 18-60 years of age, in the low socioeconomic classes, seriously mentally ill, with limited resources and social supports. Services are holistic and include counseling, case management, meditation, exercise and nutrition. The program works closely with the Adult Criminal Justice Program from the Santa Clara Mental Health and Treatment Court. Participants are court mandated to enroll in this program. Participant payment is a mix of Medi-Cal and county funds.
Supportive & Therapeutic Options Program (S.T.O.P.)
S.T.O.P. provides services for substance abuse, mental health, dual diagnosis and family counseling for youth under the age of 18. The program seeks to help clients achieve a successful and independent life. Services include comprehensive assessment, case management, group process, individual and family therapy sessions, motivational interviewing/ counseling skills, behavioral, cognitive and family therapies. Additional age-appropriate skill development is provided to ensure relapse prevention. Participants are referred by the Department of Social Services, Juvenile Probation, schools, court system, other agencies and parent and/or self referral. No payment is accepted from clients. Services are funded by the County of Santa Clara.
Ranch Victims/Offenders Program
The Ranch Victims/Offenders Program provides services to youth incarcerated at the County Juvenile Hall and the Ranch Programs. A comprehensive assessment addresses the client’s therapeutic needs as associated with sexual victimization/criminal sexual offences. Core services include group process for victims and for offenders, and individual/ family therapy sessions. Support is also provided to help build inter-relationship skills, to developing adaptive coping skills, to reduce recidivism rate of sex crimes and to assist survivors with the healing process. Participation is by referral through the Santa Clara County Juvenile Probation Department. Services are funded by the County of Santa Clara and are at no cost to the client.
SUBSTANCE ABUSE & TREATMENT
Adult Outpatient Program (AOP)
The Adult Outpatient Program is a State certified Alcohol and/ or other Drugs (AOD) program that treats stabilized episodic users whose level of dysfunction requires individualized counseling and group support. The first step for treatment is for the participant to call Gateway at 1-800-488-9919 for a level of care assessment and proper referral for treatment placement.
Deferred Entry of Judgment (DEJ)
Most nonviolent drug offenders could benefit much more from treatment and education than from jail and a criminal record. Penal Code 1000 PC – California’s Deferred Entry of Judgment (“DEJ”) program – allows just such an alternative. It’s a drug diversion that allows eligible defendants to be “diverted” out of the criminal court system and into a drug rehabilitation program.
Participants who are granted Deferred Entry of Judgment by the courts must complete 24 hours of psycho-educational classes. We offer our classes in English, Spanish and Vietnamese on eekdays and weekends. Participants may schedule an appointment by calling 408-977-1595.
We are a State licensed driving under the influence program. Our efforts are to reduce the personal and property losses caused by drivers under the influence of alcohol and/or drugs. Our Drinking Driver Program helps participants examine the arrest experience and the reasons for their arrest. It also helps them learn to make appropriate driving decisions for the future. During the introductory session, participants are asked to consider the reasons and goals of the program and its requirements for completions. Under the guidance of the manager and staff, the participant’s discus the social, medical, legal and driver safety problems cause by alcohol and other drug abuse. The program includes classroom education, screening and evaluation.
Our multicultural staff is certified by CADDTP, meeting the qualifications specified under Title 9 of the CA Code of Regulations.
