For more than a decade, the El Camino Health Community Benefit program has been a vital partner in advancing health equity through its continued support of Gardner Health Services’ Down with Diabetes Program. Their ongoing commitment, including renewed funding for the 2025–2026 fiscal year, ensures that individuals living with diabetes or at risk for the condition can access high-quality, culturally responsive care tailored to their individual health needs.
For many diabetic and prediabetic patients, the cost of care, supplies, and education presents a significant barrier. Without this grant funding, many community members would face serious challenges accessing essential diabetes management services. El Camino Health’s generous support helps remove these financial obstacles, allowing patients to receive consistent care, education, and resources that lead to improved health outcomes and long-term disease prevention.
This funding allows Gardner Health Services to provide comprehensive case management, critical medical supplies, and multilingual diabetes education for both youth and adults. Services are offered at three Gardner Health Services locations in San Jose:
- Gardner Health Center
- St. James Health Center
- CompreCare Health Center
Through this program, patients receive access to essential diabetes care services, including:
- Comprehensive health assessments
- Personalized nutrition and exercise treatment plans
- Blood tests to monitor HbA1c levels
- Individualized case management
- Food vouchers, glucose monitors, and an annual supply of test strips
- Educational materials and culturally relevant resources
- Nutrition education workshops
