El Camino Health: Community Benefit Grant Funding FY2024

For the last 8 years, the El Camino Health Community Benefit program has funded Gardner Health Services’ Down with Diabetes Program. They once again continue to support this vital initive in the our community this fiscal year (2023-2024) by providing funds to support the care and management of our diabetic and prediabetic patients. The cost of diabetes care and management can be a barrier for many patients, and this grant helps us break through this barrier for our patients which inturn results with better health outcomes.

To better serve our community, this generous grant funds case management, supplies, and bilingual care and guidance for youth and adults with or at high risk for diabetes and prediabetes. Services are provided at three of our health centers: Gardner Health Center, St. James Health Center, and Comprecare Health Center in San Jose, CA.

This grant provides:

  • Assessments
  • Nutrition and exercise prescriptive treatment plans
  • Blood tests to monitor HbA1c levels
  • Case management
  • Food vouchers, glucose monitors and an annual supply of strips
  • Educational materials and resources
  • Nutrition education workshops
