Our Lead Care Managers address all the complicated social issues that affect a person’s health.
We offer case management through the Enhanced Care Management Program for patients with the highest needs. We manage patients with chronic and high risk illnesses to achieve optimal health and to support their right to self-determination. Our patients are empowered with personalized plans to manage their medical conditions and psychosocial needs. Enrollment is subject to approval from your assigned health plan.
